Mission
The mission of Journey Across Maine Navigators is to improve cancer care for patients throughout Maine by working together as navigators and providing holistic, patient centered care through all stages of cancer care. Navigators will work together to assist patients in overcoming barriers to care, improve access to care, provide education, and work to empower all cancer patients to improve quality of life throughout the patient’s cancer journey.
Vision
The vision of the Journey Across Maine Navigators is to achieve, through effective navigation, patient-centered superior quality cancer care coordination from pre-diagnosis through survivorship/end of life throughout the state of Maine.
Goals/Objectives
To collaborate with other navigators. Maine is very rural and often patients need to travel hours for care. Some care is only provided in certain areas, but at other times the same care is offered close to home. Let’s empower patients and advocate to keep patients as close to home as possible. We want to improve quality of life for all patients throughout their cancer journey. We hope to increase access to care, remove barriers, decrease financial toxicity, decrease time toxicity, share resources, improve survivorship care planning, increase palliative referrals and improve clinical trial enrollment. Have navigators across the state work together and mentor each other.
