Target Audience This educational initiative is directed toward hematology and oncology nurse navigators, and is also recommended for patient navigators, social workers, and case managers whose focus is on cancer care and survivorship.

Educational Objectives After completing this activity, the participant should be better able to:

  • Define Triple Aim including how this initiative drives quality
  • Define the healthcare environment that is driving focus on quality and outcomes
  • Discuss the IOM reports that address the need for quality in healthcare including how this shapes practice and initiatives in oncology
  • Define evidenced-based practice, quality and performance improvement terminology
  • Discuss key drivers for evidenced-based practice and quality
  • Identify the regulatory agencies that mandate quality improvement
  • Identify evidenced-based practice models
  • Identify key quality and performance improvement tools
  • Describe the steps to develop a quality improvement project
  • Discuss how data is used for continuous improvement
  • Provide appropriate and care and counsel for patients and their families
